Terms & Conditions

These terms and conditions are effective from 1st July 2020.

For the purpose of this document, Priscilla Smith-Bizzotto and any subcontract personal trainers hired by Metamorphosis Fitness Coaching & Personal Training are here in known as The Fitness Coach and any person booking into and attending a Metamorphosis Fitness Class or PT Session, will be herein known as The Client.

GENERAL TERMS AND CONDITIONS

  • The Client certifies that:
    1. He/She is physically capable of participating in a strength, flexibility and/or aerobic training exercise program and using the equipment associated with such training; and
    2. He/She is over the age of eighteen (18) (if under the age of eighteen a guardian’s consent is required); and
    3. He/She has either (i) had a physical examination and been given a medical practitioner’s permission to participate in this training program; or (ii) decided to participate in this training program without the approval of a medical practitioner.
  • The Fitness Coach is not a medical professional and is without expertise to diagnose medical conditions or impairments. The Client agrees to promptly and fully disclose to The Fitness Coach any injury, condition or impairment which may have a deleterious effect on or be impacted by this training program and the Fitness Coach’s decision to discontinue training because of any condition which presents an adverse risk or threat to the health or safety of The Client, The Fitness Coach or others shall be conclusive.
  • If in the event of a medical issue arising either before the commencement of the physical training sessions or during coaching sessions The Fitness Coach will refer The Client to a medical practitioner/Allied Health Professional following industry best practice guideline as prescribed by Fitness Australia.
  • This training program will involve physical activity and, as appropriate, use of certain equipment and machinery. Such activities may present a risk of injury or even death.  The Client voluntarily agrees to participate in these activities and to assume all risk of personal injury, death and property damage resulting from such activities, use of equipment, machinery or public or private facilities.  The Client may opt to not undertake particular activities within the Fitness class if they deem them to be unsafe for them, in which case substitute exercises will be arranged. The Client will ensure they have adequate space to exercise safely during all Zoom sessions.
  • The Client agrees on behalf of him/herself and his/her personal representatives or heirs to release and discharge The Fitness Coach, his/her agents, representatives, successors and assigns from any and all claims or causes of actions (known and unknown) arising out of the training program including without limitation injury or loss resulting from The Client’s use of any equipment or facilities which break or malfunction.
  • Metamorphosis Fitness Coaching & Personal Training does not assume responsibility for any lost or stolen personal property. If The Client does become the victim of a theft, immediately report it to a Metamorphosis Fitness Coaching & Personal Training staff member so the matter can be logged and referred to the proper authorities.
  • No implied warranties or representations are made other than those expressly contained herein, and this document contains all the terms of the Agreement between the parties. The Fitness Coach expressly notes that results will differ for clients based upon various factors including without limitation; body type, nutrition, work ethic, etc. and no guarantees of results are possible.
  • Proper nutrition, hydration and adequate rest are an essential part to the training program. The Client must not attend training if overly fatigued or if under the influence of drugs or alcohol.  The Fitness Coach has the right to refuse any parties that are not fit to exercise.
  • Personal and group training requires a small amount of physical contact between The Fitness Coach and The Client to ensure proper technique. The fitness coach will at all times attempt to direct without contact and will ask the client for permission if at any point contact needs to be made. During Covid-19 restrictions the fitness coach will change the exercise should verbal direction not be adequate to direct the client to perform certain exercises correctly.  At no time is contact to go beyond what is relative to each exercise or outside of Client/Fitness Coach Business relationship.
  • Any information obtained during the coaching relationship is of a private and confidential nature and will be handled in accordance with the relevant privacy acts and will only be discussed to any other parties with the express written permission of The Client or where the information may result in harm to self or to others.
  • Metamorphosis Business Hours are Monday, Wednesday & Friday are 6.00am to 12.00noon, Tuesday and Thursday 6.00am-8.00am, 5.00pm-8.30pm and Sunday 8.00am-10.00am.
  • In the event of any conflict or dispute arising from the execution of these terms and conditions it is recommended to resolve the issue utilising the processes outlined in the document “Your Guide to Dispute Resolution” available as a downloadable document from the Attorney-General’s Department ; ag.gov.au

BOOKINGS, PAYMENT, ATTENDANCE & CANCELLATIONS

  1. All class bookings are to be completed within the MyPTHub App via the Calendar section by The Client.
  2. Payment must be made prior to booking. To book into a class The Client must either have pre-purchased credits in their account or pay at the time of booking using the credit card facility within the App. Class packs can be purchased via EFT, cash or via credit card in the MyPTHub. For EFT and cash transactions, the credits will be added once payment has been deposited. Please contact The Fitness Coach for payment details. Direct Debit Term memberships must be paid for via a weekly direct debit subscription within the MyPTHub.  Please refer Packages and Membership section for further terms and conditions for each Membership type.
  3. Group Fitness classes denotes all classes of 6 or more participants available within the MYPTHub App, delivered either face to face or via Zoom. It does not include 1:1 Personal training or private/specialised group fitness bookings.
  4. Group Fitness classes, as described in Item 3, cost 10 credits per person per class.  Other training or specialised classes are per private contract between clients and trainers.
  5. Booking of class attendance for the Term must be completed by The Client at the time of purchase of the Term.  In the event The Client is on a direct debit arrangement, the client must book all their classes when they receive their weekly credits.  If The Client has trouble booking in, contact should be made via the message section in the MyPTHub App or via text on 0417 324 710 within business hours. Failure to book in via the MYPTHub within this time frame will incur a small late notice administration fee or forfeit of credits.
  6. Booking of class attendance for the 5 session pass must be completed by The Client at the time of purchase. All classes are to be booked within 3 days of the purchase date of the pass.  If The Client has trouble booking in, contact should be made via the message section in the MyPTHub App or via text on 0417 324 710 within business hours. Failure to book in via the MYPTHub within this time frame will incur a small late notice administration fee.
  7. Changes to any classes due to weather or unforeseen circumstances will be advised through a WhatsApp private group message that The Client will be joined to upon signing up. Whenever possible, if a class is cancelled, the offer of the class via Zoom will be provided to all clients who have booked in.  Advance notice of cancellation or reschedule will be given at least 24 hours prior to class commencement. However, a turn in the weather or unforeseen circumstances beyond The Fitness Coaches control may only be able to be advised up to 15 mins prior to the class starting.  Regardless, in the event a class is cancelled by The Fitness Coach and cannot be conducted via Zoom, credits used to book into the class will be refunded to The Client.
  8. Zoom classes can be utilised either live or via the recording up to 7 days after the class has been completed.  If you book into a Zoom based class and cannot make the alloted time, a link to the copy of the recording will be sent to you via either text message or email.
  9. If The Client needs to cancel out of a scheduled class they have booked into, 24 hours’ notice will need to be provided. The Fitness Coach understands that sometimes unforeseeable circumstances can arise, however, must be fair and consistent with all Clients.  In the event less than 24 hours’ notice is provided The Clients credits will be forfeited for that class, unless cancellation is for medical reasons and is advised at least 6 hours prior to class commencement and a medical certificate can be provided.
  10. All classes are on a first come first serve basis. There are set numbers per class to ensure Council Permits as well government restrictions on groups are adhered to.  Clients will be turned away if over the set number.
  11. If The Client has not booked into a class in advance, they are to check MyPTHub as well the WhatsApp private message group to ensure that the class is still taking place and/or has not moved locations. Please note that phones are answered during business hours, however not during class times. Therefore, if planning to attend a class not yet booked into please check the WhatsApp messages and MyPTHub in advance before booking in/turning up!
  12. Distractions such as taking telephone calls, having children or pets present, conversing a lot during the program, etc, may affect consistent efforts and results.  To ensure maximal output and other Client enjoyment, it is requested that if a phone call must be taken it is done so away from the training session and rest breaks are utilised for longer conversations not during the workout.  For all Clients, children and pet’s safety it is asked that if pets and children are in attendance with The Client (and they are very welcome) that they set up a safe, but not unreasonable distance from the main class space during the session time.
  13. The Client is to wear appropriate athletic footwear and loose, comfortable clothing to facilitate ease of movement.
  14. The Client needs to ensure they are ready to exercise at the appointed class times, bringing with them an exercise mat or towel and a water bottle. There is to be no sharing of equipment or personal items.  Please note that due to Covid-19, no boxing classes will take place until social distancing measures are no longer required.
  15. Out of consideration for The Fitness Coaches, other clients, and for The Client’s own safety, the warm-up is an important aspect of each class. Please be aware that if The Client is more than 5 minutes late for a class, The Client may not be able to train or will miss part of the class to complete an appropriate warm up prior to commencing class.
  16. The Fitness Coach reserves the right to restrict clients to a particular class type for safety reasons; and The Fitness Coach reserves the right to turn clients away if they do not have the correct medical clearance, are not appropriately attired, are fatigued, ill or are under the influence of drugs or other substances.
  17. The Client, if pregnant can only attend General Fitness group classes up until 15 weeks pregnant. After 15 weeks The Client can attend the core, glutes and activations classes only. For Client and baby safety, The Fitness Coach must be advised of pregnancy at the first available opportunity. Attendance is subject to availability, a completed Authority to Exercise form and GP approval.
  18. Classes are not transferable to another individual and cannot under any circumstances be used by another person. If someone other than The Client booked into the class arrives at the scheduled class, they will be turned away or asked to create an account and purchase their own class pass.
  19. In line with current COVID-19 Restrictions for the State of Victoria, all Clients in attendance must adhere to all guidelines set by the State and Federal Governments.  This includes, but is not limited to 1.5mtrs social distancing, no sharing of equipment or personal items, using hand sanitiser before, during and after class, limits of gathering numbers, do not attend class if you are unwell or suffering from any of the Covid-19 symptoms. advise if attended class and have become unwell after, downloading of the CovidSafe App (please note the downloading of this app is voluntary, however it is a preference of Metamorphosis Fitness Coaching and Personal Training that you download the app to help keep people safe.
  20. By booking into a class The Client automatically agrees to all of the above Terms & Conditions.

Packages & Memberships

Term 4 Membership  # 1 – 1 x Group Fitness Class per week (paid in full)

This Membership package entitles the individual (or family group) access to one x 45 min group fitness class per week for Term 4 of the Victorian School Term, dated Sunday 4th October 2020 to Saturday 12th December, 2020, a period of 10 weeks.

Classes are currently via Zoom (live or recorded) and if Covid-19 restrictions ease, a choice of Zoom (live or recorded) or Face to Face training will become available, subject to numbers as directed by Government and Council.   Family groups cannot split across more than one class per week in total.

  1. The cost of this package in total is $130 and is to be paid in full at the time of purchase. This equates to $13.00 per week ($13.00 per class).
  2. 100 credits will be applied immediately to your account at the time of purchase. These are to be used to book 1 class per week (10 credits per week) for the duration of the 10 weeks. All classes must be nominated and booked into at the time of purchase. In the event The Client needs to reschedule their normal set class within each week, as long as 24 hours’ notice is provided, they are able to do so. If 24 hours’ notice is not provided those credits used to book the set class will be forfeited. If a Zoom class, clients will be sent the recording of that session.  Please note that if you do not pre book your class, any unused credits will expire weekly in line with the expectation of the use of 10 credits per week, they will not remain in the client’s account to be used later in the term.
  3. Credits do not accrue from Term to Term.
  4. This membership cannot be suspended.
  5. In the event you need to cancel this membership you will need to provide 14 days’ notice, as well will be required to pay 50% of the balance amount outstanding on membership. Eg: 1 month remaining, 4 weeks x $13.00 x 50% – $26.00. The cancellation fee will be deducted from your refund.
  6. For cancellation due to medical reasons, you will need to present medical certification from a general practitioner. With this medical certification a 15% administration fee will be charged on the remaining balance to discharge the membership. Eg: 1 month remaining, 4 weeks x $13.00 x 15% – $7.80. The 15% Administration fee will be deducted from your refund.
  7. A cooling off period of 3 days applies to this package. Cancelling within this time will entitle bearer to a full refund minus 15% administration costs. Cancellation must be received in writing within the 3 day period, otherwise Clause 5 cancellation details will apply.
  8. If payment defaults you will be charged a default fee of $15.00.
  9. Unused membership credits cannot be refunded, shared or transferred to any other party.
  10. In the event Covid-19 restrictions for face to face classes are lifted, all 6.45am classes will remain available via Zoom and face to face will be 9.15am Mon, Wed and Thursday. Sunday’s class will remain in Zoom until numbers are lifted to be able to cover off all regular Sunday clients. Credits will be available to be utilised for either live or recorded Zoom or face to face classes.  If opting for face to face classes, it is noted that in accordance with government guidelines, restrictions on numbers will be in place and for this reason bookings in advance will be essential, places in class will not be guaranteed until your booking is in place.

Term 4 Membership – 2 x Group Fitness Classes per week (paid in full)

This Membership package entitles the individual (or family group) access to two x 45 min group fitness classes per week for Term 4 of the Victorian School Term, dated Sunday 4th October 2020 to Saturday 12th December, 2020, a period of 10 weeks.

Classes are currently via Zoom (live or recorded) and if Covid-19 restrictions ease, a choice of Zoom (live or recorded) or Face to Face training will become available, subject to numbers as directed by Government and Council.   Family groups cannot split across more than two classes per week in total.

  1. The cost of this package in total is $225 and is to be paid in full at the time of purchase. This equates to $22.50 per week ($11.25 per class).
  2. 200 credits will be applied immediately to your account at the time of purchase. These are to be used to book 2 classes per week (20 credits per week) for the duration of the 10 weeks. All classes must be nominated and booked into at the time of purchase. In the event The Client needs to reschedule their normal set classes within each week, as long as 24 hours’ notice is provided, they are able to do so. If 24 hours’ notice is not provided those credits used to book the set class will be forfeited. If a Zoom class, clients will be sent the recording of that session.  Please note that if you do not pre book your classes, any unused credits will expire weekly in line with the expectation of the use of 20 credits per week, they will not remain in the client’s account to be used later in the term.
  3. Credits do not accrue from Term to Term.
  4. This membership cannot be suspended.
  5. In the event you need to cancel this membership you will need to provide 14 days’ notice, as well will be required to pay 50% of the balance amount outstanding on membership. Eg: 1 month remaining, 4 weeks x $22.50 x 50% – $45.00. The cancellation fee will be deducted from your refund.
  6. For cancellation due to medical reasons, you will need to present medical certification from a general practitioner. With this medical certification a 15% administration fee will be charged on the remaining balance to discharge the membership. Eg: 1 month remaining, 4 weeks x $22.50 x 15% – $13.50. The 15% Administration fee will be deducted from your refund.
  7. A cooling off period of 3 days applies to this package. Cancelling within this time will entitle bearer to a full refund minus 15% administration costs. Cancellation must be received in writing within the 3 day period, otherwise Clause 5 cancellation details will apply.
  8. If payment defaults you will be charged a default fee of $15.00.
  9. Unused membership credits cannot be refunded, shared or transferred to any other party.
  10. In the event Covid-19 restrictions for face to face classes are lifted, all 6.45am classes will remain available via Zoom and face to face will be 9.15am Mon, Wed and Thursday. Sunday’s class will remain in Zoom until numbers are lifted to be able to cover off all regular Sunday clients. Credits will be available to be utilised for either live or recorded Zoom or face to face classes.  If opting for face to face classes, it is noted that in accordance with government guidelines, restrictions on numbers will be in place and for this reason bookings in advance will be essential, places in class will not be guaranteed until your booking is in place.

Term 4 Membership – 4 x Group Fitness Classes per week 

This Membership package entitles the individual (or family group) access to 4 x 45 min group fitness class per week for Term 4 of the Victorian School Term, dated Sunday 4th October 2020 to Saturday 12th December, 2020, a period of 10 weeks.

Classes are currently via Zoom (live or recorded) and if Covid-19 restrictions ease, a choice of Zoom (live or recorded) or Face to Face training will become available, subject to numbers as directed by Government and Council.   Family groups cannot split across more than four classes per week in total.

  1. The cost of this package in total is $330 and is to be paid in full at the time of purchase. This equates to $33.00 per week ($8.25 per class).
  2. 400 credits will be applied immediately to your account at the time of purchase. These are to be used to book 4 classes per week (40 credits per week) for the duration of the 10 weeks. All classes must be nominated and booked into at the time of purchase. In the event The Client needs to reschedule their normal set classes within each week, as long as 24 hours’ notice is provided, they are able to do so. If 24 hours’ notice is not provided those credits used to book the set class will be forfeited. If a Zoom class, clients will be sent the recording of that session.  Please note that if you do not pre book your classes, any unused credits will expire weekly in line with the expectation of the use of 40 credits per week, they will not remain in the client’s account to be used later in the term.
  3. Credits do not accrue from Term to Term.
  4. This membership cannot be suspended.
  5. In the event you need to cancel this membership you will need to provide 14 days’ notice, as well will be required to pay 50% of the balance amount outstanding on membership. Eg: 1 month remaining, 4 weeks x $33.00 x 50% – $66.00. The cancellation fee will be deducted from your refund.
  6. For cancellation due to medical reasons, you will need to present medical certification from a general practitioner. With this medical certification a 15% administration fee will be charged on the remaining balance to discharge the membership. Eg: 1 month remaining, 4 weeks x $33.00 x 15% – $19.80. The 15% Administration fee will be deducted from your refund.
  7. A cooling off period of 3 days applies to this package. Cancelling within this time will entitle bearer to a full refund minus 15% administration costs. Cancellation must be received in writing within the 3 day period, otherwise Clause 5 cancellation details will apply.
  8. If payment defaults you will be charged a default fee of $15.00.
  9. Unused membership credits cannot be refunded, shared or transferred to any other party.
  10. In the event Covid-19 restrictions for face to face classes are lifted, all 6.45am classes will remain available via Zoom and face to face classes will be 9.15am Mon, Wed and Thursday. Sunday’s class will remain in Zoom until paticipant numbers are lifted to be able to cover off all regular Sunday clients. Credits will be available to be utilised for either live or recorded Zoom or face to face classes.  If opting for face to face classes, it is noted that in accordance with government guidelines, restrictions on numbers will be in place and for this reason bookings in advance will be essential, places in class will not be guaranteed until your booking is in place.

Term 4 Membership DD – 1 x Group Fitness Classes per week, weekly direct debit, lock in contract

This membership covers access to 1 Group Fitness class (10 credits) every week for a period of 10 weeks – Sunday 4th October to Saturday 12th December.

  1. The cost of this package in total is $140 and is direct debited at $14.00 per week ($14.00 per class) over a 10 week period from a credit card nominated by yourself via the MYPTHub App.
  2. 10 credits will be applied immediately to your account at the time of purchase . The class for that week must be nominated and booked into in advance. In the event you do not utilise the 10 credits for that week, any unused credits remaining from that week’s 10 credit allocation will expire from your account at the end of the week. In the event The Client needs to reschedule their normal set class within each week, as long as 24 hours notice is provided they are able to do so. If you fail to provide notice, your credits will be forfeited. 10 new credits will be applied in line with each direct debit. Please note that if you do not pre book your classes, any unused credits will expire weekly in line with the expectation of the use of 10 credits per week, they will not accrue in the account to be used later in the 10 week block.
  3. Credits do not accrue week to week or from Term to Term.
  4. This membership cannot be suspended.
  5. For cancellation due to medical reasons, you will need to present medical certification from a general practitioner. With this medical certification a 15% administration fee will be charged on the remaining balance to discharge the membership.
  6. A cooling off period of 3 days applies to this package. Cancelling within this time will entitle bearer to a full refund minus 15% administration costs. Cancellation must be received in writing within the 3 day period, otherwise Clause 7 cancellation details will apply.

  10. In the event Covid-19 restrictions for face to face classes are lifted, all 6.45am classes will remain available via Zoom and face to face will be 9.15am Mon, Wed and Thursday. Sunday’s class will remain in Zoom until numbers are lifted to be able to cover off all regular Sunday clients. Credits will be available to be utilised for either live or recorded Zoom or face to face classes.  If opting for face to face classes, it is noted that in accordance with government guidelines, restrictions on numbers will be in place and for this reason bookings in advance will be essential, places in class will not be guaranteed until your booking is in place.

Term 4 Membership DD – 2 x Group Fitness Classes per week, weekly direct debit, lock in contract.

This membership covers access to 2 Group Fitness classes (20 credits) every week for a period of 10 weeks – Sunday 4th October to Saturday 12th December.

  1. The cost of this package in total is $240 and is direct debited at $24.00 per week ($12.00 per class) over a 10 week period from a credit card nominated by yourself via the MYPTHub App.
  2. 20 credits will be applied immediately to your account at the time of purchase . The classes for that week must be nominated and booked into in advance. In the event you do not utilise the 20 credits for that week, any unused credits remaining from that week’s 20 credit allocation will expire from your account at the end of the week. 20 new credits will be applied in line with each direct debit. Please note that if you do not pre book your classes, any unused credits will expire weekly in line with the expectation of the use of 20 credits per week, they will not accrue in the account to be used later in the 10 week block. In the event The Client needs to reschedule their normal set class within each week, as long as 24 hours notice is provided they are able to do so. If you fail to provide notice, your credits will be forfeited. If booked into a Zoom class you will receive a copy of the recording.
  3. Credits do not accrue week to week or from Term to Term.
  4. This membership cannot be suspended.
  5. For cancellation due to medical reasons, you will need to present medical certification from a general practitioner. With this medical certification a 15% administration fee will be charged on the remaining balance to discharge the membership.

Term 4 Membership DD – 4 x Group Fitness Classes per week, weekly direct debit, lock in contract

Term 4 Membership – 4 x Group Fitness Classes per week, weekly direct debit, 10 week lock in contract

This membership covers access to 4 Group Fitness classes (40 credits) every week for a period of 10 weeks – Sunday 4th October to Saturday 12th December.

  1. The cost of this package in total is $350 and is direct debited at $35.00 per week ($8.75 per class) over a 10 week period from a credit card nominated by yourself via the MYPTHub App.
  2. 40 credits will be applied immediately to your account at the time of purchase . The classes for that week must be nominated and booked into in advance. In the event you do not utilise the 40 credits for that week, any unused credits remaining from that week’s 40 credit allocation will expire from your account at the end of the week. 40 new credits will be applied in line with each direct debit. Please note that if you do not pre book your classes, any unused credits will expire weekly in line with the expectation of the use of 40 credits per week, they will not accrue in the account to be used later in the 10 week block. In the event The Client needs to reschedule their normal set class within each week, as long as 24 hours notice is provided they are able to do so. If you fail to provide notice, your credits will be forfeited. If booked into a Zoom class you will receive a copy of the recording.
  3. Credits do not accrue week to week or from Term to Term.
  4. This membership cannot be suspended.
  5. For cancellation due to medical reasons, you will need to present medical certification from a general practitioner. With this medical certification a 15% administration fee will be charged on the remaining balance to discharge the membership.
  6. A cooling off period of 3 days applies to this package. Cancelling within this time will entitle bearer to a full refund minus 15% administration costs. Cancellation must be received in writing within the 3 day period, otherwise Clause 7 cancellation details will apply.
  7. In the event you need to cancel this membership you will need to provide 14 days notice as well be required to pay 50% of the balance amount outstanding on the membership. Eg: 1 month remaining, 4 weeks x $24.00 x 50% – $70.00. Cancellation will not be applied until the cancellation fee is received.
  8. If payment defaults you will be charged a default fee of $15.00.
  9. Membership credits cannot be shared or transferred to any other party.

In the event Government Covid-19 restrictions for face to face classes are lifted, 6.45am classes will remain available via live or recorded Zoom workouts for the whole family and face to face will be 9.15am Mon, Wed and Thursday. Credits can be utilised for either live or recorded Zooms or face to face classes.

1 X Fitness Class Pass (10 credits)

  1. The cost of a 1 x Group Fitness Class pass is $15.00 ($15.00 per class) and is to be paid in full at time of purchase.
  2. The bearer of the 1 x Group Fitness Class pass is entitled to attend 1 x 45 minute group fitness classes conducted by Metamorphosis Fitness Coaching and Personal Training either face to face or via Zoom.
  3. Booking are essential and all classes must be booked at the time of purchased and utilised within a 7 days period from the date of purchase.
  4. The 1 x Fitness Class Pass will expire 7 days from date of purchase and any unused credits will be forfeited.
  5. In the event the credits cannot be utilised within the 7 days due to valid medical reasons, written notification, along with medical certification must be provided to the owner no less than 5 business days prior to the pass being due to expire to receive an extension.
  6. Membership credits cannot be refunded, shared or transferred to any other party.
  7. 5 X Fitness Class Pass (50 credits)

  1. The cost of a 5 x Group Fitness Class pass is $70.00 ($14.00 per class) and is to be paid in full at time of purchase.
  2. The bearer of the 5 x Group Fitness Class pass is entitled to attend 5 x 45 minute group fitness classes conducted by Metamorphosis Fitness Coaching and Personal Training either face to face or via Zoom.
  3. Booking are essential and all classes must be booked at the time of purchase and utilised within a 30 days period from the date of purchase.
  4. The 5 x Fitness Class Pass will expire 30 days from date of purchase and any unused credits will be forfeited.
  5. In the event the credits cannot be utilised within the 30 days due to valid medical reasons, written notification, along with medical certification must be provided to the owner no less than 5 business days prior to the pass being due to expire to receive an extension.
  6. Unused Membership credits cannot be refunded, shared or transferred to any other party.
